Tracey Elizalde, M.S., C.R.N.P.,  Tracey obtained her initial nursing degree (R.N.) from Frederick Community College in 1987. She worked as a hospital nurse in emergency and intensive care at University of Maryland Shock Trauma Center until 1992. She was a hospice/home health nurse until 1997. During this period of time she obtained her Bachelor's Degree (B.S.N.) at University of Maryland and then a Master of Science degree specializing in Family Practice completed in 1997. Tracey also completed an additional one-year internship in gynecology and women's health. She has been in private practice since 1997.

 

Gwenn Leatherman, M.S., C.R.N.P. Gwenn and her family have been residents of Boonsboro since 2010. Gwenn completed her nursing degree, Registered Nurse (R.N.) from Frederick Community College in 2009. As an RN, she previously worked in the emergency room, interventional radiology, PACU, Med- Surgical unit and long-term care. She obtained her Bachelors Degree (B.S.N.) at University of Notre Dame of MD in 2012. In 2016 she completed her Master of Science degree specializing in Family Practice at Shenandoah University. As an FNP she has experience in internal medicine, long-term care, hospice and assisted living. Currently, Gwenn is enrolled in the Doctorate of Nursing Practice (D.N.P.) program at Shenandoah University.

Erin Foutch, MSN, C.R.N.P. Erin began her career in nursing as a GNA at Reeders Memorial Home in Boonsboro in 1999. She graduated in 2004 as a Registered Nurse from Hagerstown Community College and then received her BSN from Shepherd University in 2010. The focus of her RN career was emergency/trauma. February of 2019, Erin completed her Family Nurse Practitioner degree from Walden University and is AANP certified. Erin considers it a privilege to help people live their best life through holistic, evidence-based care. 

Julie Miller, M.A., Patient Coordinator. Julie graduated from Washington County Technical High 2010 where she received her CNA certificate.  Started at South Mountain Health in September 2010.  She is in the process of going back to school for RN, then eventually for CRNP. Julie enjoys helping others and in her free time she loves spending time with her children.

 

 

Tamara White, Business Manager. Tamara is a graduate of Mansfield University, having completed the Robert Packer Hospital's Respiratory Therapy Program. She has experience as a homecare RT as well as an RT in the hospital setting. Tamara also worked in the insurance industry as a claims and customer service manager for over a decade. She has been a resident of Sharpsburg for thirty two years.

 

 

Heather Branch, Administrative and Billing Specialist.  Heather graduated from Washington County Technical High in 2009.  She went to HCC for a few years taking general education classes, unknowing of what she really wanted to study.  She started at South Mountain Health in February 2011.  In her free time, she enjoys fishing, gardening and spending time with her husband, friends and family.
