South Mountain Community Health was established in 2004 with a mission to assist our patients in meeting their health care needs and goals. We serve all people regardless of health insurance coverage status. We are proud to be active members of our community and loyal supporters of many local activities and youth sports.
It has always been our mission to serve ALL who come though our doors, regardless of financial or insurance status. Some of our uninsured patients may need referrals, testing, and prescription assistance outside of our office that they are unable to afford. We would like to thank the patients that have given donations in the past and anyone who has donated to our cause. We will always provide services to all here in the office, but we could use your help so that we can continue to offset the costs that accumulate from services outside of the office.
