Tracey Elizalde, M.S., C.R.N.P., Tracey obtained her initial nursing degree (R.N.) from Frederick Community College in 1987. She worked as a hospital nurse in emergency and intensive care at University of Maryland Shock Trauma Center until 1992. She was a hospice/home health nurse until 1997. During this period of time she obtained her Bachelor's Degree (B.S.N.) at University of Maryland and then a Master of Science degree specializing in Family Practice completed in 1997. Tracey also completed an additional one-year internship in gynecology and women's health. She has been in private practice since 1997.
Jenice Palachick, M.S., C.R.N.P. Jenice obtained her Associate's Degree in nursing from Penn College in 2003. She worked in critical care in different hospitals in Baltimore, MD. She then obtained her Bachelor's Degree (B.S.N.) at University of Maryland in 2006 while continuing to work in the Medical ICU at Johns Hopkins Bayview Hospital. In 2009 she graduated from Johns Hopkins School of Nursing with MSN in family medicine. She has worked in primary care since then. She joined South Mountain Health in October 2014. In her free time she enjoys spending time with her friends & family and various outdoor activities.
Julie Miller, M.A. Julie graduated from Washington County Technical High 2010 where she received her CNA certificate. Started at South Mountain Health in September 2010. She is in the process of going back to school for RN, then eventually for CRNP. Julie enjoys helping others, and in her free time she loves spending time with her children.
Erin Collinson, RMA/CPT. Erin obtained her Associate's Degree in Applied Science from Hagerstown Community College in 2015, as well as her Phlebotomy Certification in 2014. She did her clinicals at John's Hopkins Community Physicians practice in 2015. Before getting her degree she worked with the mentally disabled clients, which influenced her to get her degree in the medical field. In her free time she loves being with her family, husband and children.
Tamara White, Business Manager. Tamara is a graduate of Mansfield University, having completed the Robert Packer Hospital's Respiratory Therapy Program. She has experience as a homecare RT as well as an RT in the hospital setting. Tamara also worked in the insurance industry as a claims and customer service manager for over a decade. She has been a resident of Sharpsburg for twenty six years.
Heather Branch, Front Office Manager. Heather graduated from Washington County Technical High in 2009. She went to HCC for a few years taking general education classes, unknowing of what she really wanted to study. She started at South Mountain Health in February 2011. In her free time, she enjoys fishing, gardening and spending time with her husband, friends and family.
Kim Shafer, Front Office. Kim graduated from Middletown High School. She studied to be a CMA at Frederick County Technical Center. She worked in a nursing home as a CMA for eight years. She has many years of experience in the medical field. She has always enjoyed helping others. In her free time she enjoys spending time with her kids and her husband.
